Areyou an enthusiastic Clinical Pharmacist looking to make a real difference inpatient care? Join our dynamic Primary Care Network team, where you will play avital role in optimising medicines use, improving outcomes for patients, andreducing the demand on GPs. In this role, you will utilise your clinicalexpertise to provide structured medication reviews, manage long-termconditions, and support our pharmacy technicians and administrative staff todeliver safe and effective medicines management. You will also have theopportunity to lead and contribute to innovative, service-driven projects thatenhance care pathways and drive improvements across the network. Main duties of the job

The ClinicalPharmacist will work as an integral part of the Primary Care Network (PCN) teamto optimise medicines use, improve patient outcomes, and reduce demand on GPsthrough the provision of expert clinical advice, structured medication reviews,and management of long-term conditions. The post holder will support andsupervise pharmacy technicians in delivering safe and effective medicinesmanagement processes, as well as work closely with the administrative team tostreamline prescribing-related tasks and queries. In addition, the ClinicalPharmacist will contribute to the development and implementation ofservice-driven projects that enhance patient care, improve system efficiencies,and align with local and national priorities across the PCN.

Job responsibilities

Clinical

Frequentlyinvolves direct patient contact via face to face, video or telephone. Mayinclude domiciliary or care home visits.

Competentlyrun patient clinics, organising own workload and processes involved to ensure timelypatient follow-up and medicine optimisation.

Participatein and lead on long term condition management plans, always considering a holisticapproach within your area of competence.

Completestructured medication reviews (SMRs).

Priorto a review ensure any necessary monitoring has been established / encouragedor arrange it. As competence grows this will involve ordering and then laterreviewing blood tests.

Confidentlyreview and, in collaboration with other HCPs, competently make changes totherapy if required. Be prepared to refer for additional care, information orsupport as necessary.

Supportin improving patient understanding and education. This could involve groupsessions or the production of health promotional information and materials.

Addressday to day issues raised by patients / staff or other HCPs in relation tomedicine management. E.g. alternatives for out-of-stock items, query need foritems not on repeat prescription and overseeing requests for prescribing fromother professionals.

Providesupport, guidance and assistance as able to other members of the pharmacy team

Ensureclear, concise and appropriate documentation at all times

Person Specification

Qualifications

  • Mandatory registration with General Pharmaceutical Council (GPhC)
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
  • Independent prescriber or working towards/intent of gaining an independent prescribing qualification
  • Completion of CPPE PCPEP (or equivalent)

Experience

  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
  • Excellent interpersonal, influencing and negotiating skills.
  • Excellent written and verbal communication skills
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g., patients)
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
  • Computer literate with an ability to use the required GP clinical systems (desirable) and Microsoft office packages (essential)
  • Able to analyse and interpret prescribing data.
  • Has attention to detail, able to work accurately, identifying errors quickly and easily.
  • Able to effectively manage allocated resources.
  • Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines.
  • Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems.
