Job summary
The West Devon Core Adult Mental Health Team are looking for an experienced and skilled practitioner to join our service. This is an exciting time to join the core team as under new management we continue to build close links with our primary care colleagues under the Community Mental Health Framework. The job will require a mix of care planning, brief intervention, multi-agency liaison and risk managemnt. The team provied regualar managerial and clicnial supervison and training opportunities for carrer devolopment

This will include recovery co-ordination (design and co-ordination of treatment packages that are personalised and evaluated in line with payment by results). To provide statutory Adult Social Care, Care Management functions (assessment, care planning and reviewing). The liaison with a wide range of statutory and non-statutory agencies is central to this role.
