Job Title: Adults Social Worker
Authority: Manchester City Council.
Contract: 35 hours per week
Social worker required for the hospital discharge to assess south team.
About the role:
To support with reviewing care needs for citizens on discharge from hospital. To complete Care Act assessments and Mental Capacity assessments where required.
To support all of the pathways out of hospital including reviewing care packages in the community, supporting discharges from intermediate care beds and reviewing citizens discharged into pathway 3 settings such as residential and nursing beds.
To also support crisis response to prevent citizens at risk of hospital admission and our hospital at home service.
We cover weekends approx 1 in 4.
