Our practice is looking for an enthusiastic, conscientious, Community Matron/Care Home Nurse. To provide case management to patients with complex needs. Working as part of the Community Support Team based in practice (Senior Community Matron, Care Home Nurse, Care Co-ordinators).

We encourage and help with the professional development of staff within the organisation with training as needed.

This is an exciting opportunity to work in a friendly, supportive, forward thinking training practice, working from a modern purpose built premises delivering high quality health care., The post holder will assess the patients’ holistic needs, communicating complex and sensitive information to the patients and carers relating to their assessment, diagnosis, prognosis and treatment plan.

Experience

Essential

  • Min. 5 years post registration

  • Demonstrable experience in autonomous clinical/advanced clinical practice, prioritisation and prescribing.

  • Experience of ordering and interpreting diagnostics.

  • Experience of team working and leadership qualities.

Desirable

  • Experience of working in the community

  • Experience in General Practice, OOH or emergency care.

  • Experience in management of long term conditions.

  • Experience of frailty & management of care home patients

  • Experience of undertaking in-practice diagnostics.

Essential

  • Will be able to make own travel arrangements to patients’ homes, clinics, base and meetings etc, as required.

  • Must hold a full and valid driving licence and access to a vehicle.

Qualifications

Essential

  • Registered General Nurse (NWC)

  • BSc or MA in Advanced Clinical Practice and/or relevant experience.

Desirable

  • Advanced physical assessment & Consultation (APACS)

  • Teaching/mentorship qualification.

  • Minor illness/injury qualification.

  • Specialist courses relevant to primary care.

  • Independent Prescriber, * Current primary care policies, national legislation, GP contracts, clinical governance,

  • QOF and clinical commissioning.

  • National and local prescribing standards.

  • Good working knowledge of prescribing complaints seen in General Practice.

  • Accurately able to interpret diagnostics.

  • Understanding accountability arising from NMC Code of Professional Conduct and medico-legal aspects of the role.

  • Equality and diversity

  • Working knowledge of community services

  • Advanced knowledge of diagnostics

  • Training and recent updates on care of unwell adults ,

  • Adult safeguarding training.

  • Able to compile clinical policies and protocols and introduce them in practice.
