Go back Black Country Healthcare NHS Foundation TrustNurse – Band 6 Clozapine & Physical Health Lead NurseThe closing date is 30 July 2025This is a new specialist Physical Health Team developing within Adult Community Mental Health Services in Black Country Healthcare NHS Foundation TrustThe Trust has undergone a Community Transformation Programme to improve the physical health outcomes for our patients with direction and standards issued by NHS England and NICE guidelines.You will undertake cardio-metabolic monitoring in Out-patient Clinics and complete occasional home visits.Main duties of the jobThe role of the Band 6 Clozapine & Physical Health Nurse is to facilitate clinics within Out-patients, complete occasional home visits to undertake annual physical health checks for patients and monitoring of Clozapine, Lithium, or High-dose Antipsychotics. You will monitor physiological measurements, complete ECG\’s and take bloods (training will be provided).You will lead a one-stop Clozapine Clinic, ensuring correct monitoring for all Clozapine patients; blood analysing, physical observations and side effect monitoring. CPMS training will be provided.We are seeking skilled, reflective and experienced clinician (RMN or RGN) who are able to form good working relationships and will further embed best practice, ensuring parity of esteem to adults with SMI.You will be flexible, creative and adaptable, will be committed to working in partnership with patients and their carers, working alongside the multi-disciplinary team. Applicants must have excellent communication skills, computer skills, time management and organisations skills are essential. A genuine interest in improving the physical health for patients and should be able to form good working relationships with the multi-disciplinary team to deliver a safe, high quality and effective service. You will need to use your clinical knowledge and skills to prioritise care, act and refer accordingly. Team work is our strength and our passion is to deliver high quality care and with a supportive work environment.Job responsibilitiesTo provide support, advice, care management and structured interventions to service users who present with physical health issues secondary to severe and enduring mental health problems within a team based approach.To provide a high standard of delivery of care within the Physical Health and Clozapine Clinics including the monitoring of patients on Lithium and High dose antipsychoticsTo work, as a professional member of the nursing team within the Core Community Service setting.The post holder is responsible for the assessment of care needs development, implementation and evaluation of programmes of care and will be required to teach nursing and non-nursing staff including basic and post basic students.The post holder will be expected to be involved in the leadership of the clinical team and will carry out all appropriate care interventions and may be designated to take charge of the department/caseload or speciality in which she/he is placed in the absence of the person who has continuing responsibility.The post holder will assist the Clozapine and Physical Health Team Manager in developing and maintaining the quality of the department, including monitoring high standards of client care and developing areas of clinical excellence/best practice and a therapeutic & caring culture.To support the service in developing partnership working and physical health care pathways in order to improve the health of service users and the knowledge of carers/relatives as appropriate.Person SpecificationEssential/DesirableRMN/RGNEvidence of study/training in relevant specialist subject care – Physical health careExperience of working with people within area of specialityWorking in a multi-disciplinary teamAbility to prioritise workloadAbility to support and supervise staffKnowledge of Clozaril / Lithium and high dose anti psychotic medication monitoringAwareness of the importance of the promotion of physical health care with patient groupExperience of measuring and recording physiological measurements in routine presentations with service usersTrained in and experience of Venepuncture with service users (or willing to undertake the training)Trained or willing to undertake POCHI trainingTrained or willing to undertake ECG monitoringAwareness of point of care testing for ClozapineUnderstanding the organisational framework in which the service operatesFull Driving LicenceNon Medical PrescribingEvidence of Post Registration trainingDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.Black Country Healthcare NHS Foundation TrustAddressQuayside House – Community / OutpatientsQuayside House – Community / Outpatients #J-18808-Ljbffr
