We are seeking two empathetic and driven Senior Social Worker\’s to join our Urgent Care Acute and Prisons Service in Morecambe Bay on a full time, permanent basis.

Key Responsibilities
- Provide a single point of access to community intermediate care services, including reablement and residential rehab, 7 days a week.
- Support hospital discharges and work with the wider multi-disciplinary team to prevent hospital admissions.
- Utilize the Discharge to Assess model for assessments away from the hospital environment.
- Conduct holistic, face-to-face Care Act Assessments, reviews, and support planning using a strengths-based approach.
