Applications are invited from suitably experienced Clinical Pharmacists to join the Pharmacy team as Interface Pharmacist within the Health Service on the beautiful Island of Guernsey, in the Channel Islands.

This is a newly created post that will impact practice across all specialties on Guernsey in both Acute/Hospital and Community/Primary Care settings.

Joining a team of 7 Band 8A Pharmacists, reporting to the Chief Pharmacist, you will:

  1. Provide expert advice on medicines at the healthcare interface, developing strong stakeholder relationships to ensure the safe, cost-effective use of medicines aligned with the Partnership of Purpose.
  2. Act as the deprescribing champion for the organization, empowering hospital and community pharmacists to support deprescribing initiatives.
  3. Design, develop, and implement integrated medicines processes across primary, secondary, care homes, and other settings to ensure safe, effective, and cost-effective medication use, with emphasis on multi-morbidity, polypharmacy, and deprescribing.
  4. Create processes to ensure timely and accurate information exchange between Primary and Secondary care during patient admissions and discharges.

You will liaise with:

  • The Chief Pharmacist, Deputy Chief Pharmacist, Prescribing Advisor, Pharmacy Hospital Services Manager, Head and Lead Pharmacy Technicians, and other pharmacy staff.
  • Management staff, external agencies, patient groups, and the pharmaceutical industry.
  • Medical, Nursing, and Pharmacy staff in primary and secondary care.

Please note: Guernsey has adopted the AfC banding system but applies its own salary scales. The Band 8A salary range from 1st Jan is £70,723 to £84,752, plus an annual bonus of £1,605 and higher out-of-hours rates if applicable. Annual leave ranges from 36 to 42 days, with full credit for NHS service for leave and sick leave.

The integrated health service provides diverse services, including acute hospital care and community support. The pharmacy team is supported by modern equipment and high-standard healthcare professionals.

Person requirements:

  • Qualified GPhC-registered Pharmacist.
  • At least three years of UK post-registration practice experience.
  • Experience in Medicines Optimisation.
  • Evidence of engagement in clinical governance.
  • Ability to act independently with appropriate supervision.
