What We're Looking For:A minimum of 3 years' post-qualified experience as a social worker, specifically within adult social care, with experience in CHC assessments or reviews.Current and valid Social Work England registration is essential.A strong understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care, the Care Act 2014, and other relevant adult social care legislation.Proven ability to undertake comprehensive assessments, coordinate complex care packages, and conduct statutory reviews, ensuring person-centred outcomes.Excellent communication and negotiation skills, with the ability to work effectively with individuals, families, healthcare professionals, and partner agencies.
