Role Details

Support Coordinator (Hospital Discharge) – Salary £25,838 and benefits including a Health Cash Plan.

Permanent, full‑time (37.5 hpw), Monday to Friday 9am–5pm.

Location: Musgrove Park Hospital, Taunton and The Link, Yeovil.

What You’ll Do

  • Visit wards at Musgrove Park Hospital and the community hub The Link in Yeovil one day per fortnight.
  • Partner with local hospitals and housing teams to reduce the average length of stay on inpatient wards for customers who are homeless or rough sleeping.
  • Create person‑centred discharge plans with and for customers, ensuring they are reviewable and sustainable long term.
  • Collaborate with health, housing, and other services to facilitate a smooth transition from hospital to accommodation for customers with complex needs.

You have

  • Passion to support customers to live their best life, working collaboratively with an eye for detail.
  • Experience creating person‑centred support plans and supporting colleagues in working to those plans.
  • Knowledge of housing law and legislation.
  • Experience coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure, and have a resilient approach.

The practical bits

  • Working days Monday to Friday 9am–5pm, with flexibility for customer needs.
  • Ability to use technology for creating and updating support plans, completing online learning and collaborating with colleagues.
  • You\’ll need an Enhanced DBS check, which we will pay for.
  • A vehicle insured for business use and a valid driving licence (business mileage will be paid).
