Job Introduction
This is a rewarding role within the Reablement Service, a short-term service that supports people in their own home or in Residential or Nursing Homes, aiming to maximise and maintain independence through rehabilitation and support. You will be part of a multidisciplinary team including Reablement Therapy Workers, Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners, Care Coordinators and Administrative support. The team collaborates with local GPs and community services such as District Nurses and Social Workers.
Travel around Bath and North East Somerset is required; a Full UK Driving License and access to a vehicle for work purposes are essential. The role is full-time based at St Martinβs Hospital, Bath. Service hours are 8:00β22:00 with core hours typically 8:00β16:00, and 9:00β18:00 or 8:00β14:00 once weekly. The service operates seven days a week and staff work weekends and bank holidays within a team rota. Enhancements for unsocial hours are paid in line with AfC rates.
Main Responsibilities
- Carry out therapeutic assessment, diagnosis and treatment with patients using person-centred goals.
- Provide an evidence-based Occupational Therapy service of a high standard in a community setting through comprehensive assessments, reassessments, care planning, rehabilitation and therapeutic care and support.
- Hold responsibility for own caseload while working with rehabilitation therapy workers to deliver therapeutic programmes.
- Provide effective supervision to junior Occupational Therapists, Therapy Assistant Practitioners and Reablement therapy workers.
