- Assist adults with complex needs, ensuring their safety and well‑being through necessary safeguarding interventions.

Requirements:
- A Social Work qualification (BA/MA Social Work, DipSW, CQSW or equivalent).
- Current registration with Social Work England.
- Proven experience in hospital discharge planning or within an adult social care setting.
- Strong understanding of legislation such as the Care Act 2014 and Mental Capacity Act 2005.
- Proficiency in managing a caseload efficiently under pressure.
- Demonstrable skills in communication, assessment, and report writing.
