We are hiring for a full-time Home Health Patient Care Manager RN Hybrid to join our passionate team! This PCM will spend half their time in the office and the other half in the field seeing patients.
Salary: $85,000 – $88,000

Responsibilities
The Home Health Patient Care Manager and RN Hybrid is responsible for supervising and coordinating clinical services, providing and directing nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinates and supervises an interdisciplinary team to ensure high-quality care to home health patients in accordance with physician plans of care and all regulations.
- Receives referrals, assigns clinicians and therapists for timely patient evaluation, and authorizes start of care visits.
- Coordinates patient home health benefits, medical necessity, and insurance approvals.
- Ensures patient needs are continually assessed, and care is individualized, appropriate, and meets eligibility criteria and physician orders.
- Manages and documents communications with physicians, clinicians, patients, and referral sources, including new orders and patient updates.
- Coordinates care with all disciplines and providers, follows up on tests and significant changes, and communicates updates to clinicians.
- Schedules and facilitates case conferences and reports, ensuring effective interdisciplinary communication.
- Assists with patient transfer and discharge processes.
- Follows up on On-Call events daily and ensures documentation is completed properly and timely.
- Reviews clinician notes weekly, evaluates service delivery, and takes corrective actions as needed.
- Enters incidents into the Risk Management system as per policy.
- Assists in onboarding new personnel and provides leadership to clinical team members.
- Delivers high-quality clinical services within scope, meets productivity standards, and completes comprehensive assessments.
- Supervises care provided by aides and practical nurses, providing instruction and supervisory visits.
- Develops and revises care plans in collaboration with physicians and health professionals.
- Communicates effectively with staff, physicians, patients, and families to ensure quality care.
Education and Experience
- Current RN licensure in the state of practice
- Current CPR certification
- Valid Driver\’s License, vehicle insurance, and access to transportation
