About the Role

Are you passionate about supporting young people transitioning from care into adulthood? Do you want to be part of a team dedicated to delivering high-quality support to care leavers? If so, this is an exciting opportunity for a Practice Manager to join the Pathways Leaving Care Service, responsible for delivering statutory services to young people aged 18+ who have left care.

The service currently consists of two pods, each led by a Practice Manager supporting a team of Pathways (Personal) Advisers. You will work closely with colleagues across the council and partner agencies to ensure our care-experienced young people receive the support they need to thrive and achieve positive outcomes.

Key Responsibilities

  • Supervise and support staff within the Pathways Leaving Care Service, including leading a team of up to 6 Pathway Advisors and social workers/student social workers as needed.
  • Support young people leaving care to transition successfully into independent living and adulthood.
  • Participate in regular training and team reflect meetings, operating within Calderdale\’s systemic Pod model.
  • Drive continuous improvement to achieve an Outstanding service.

Candidate Requirements

  • Recognised social work qualification in a relevant field.
  • Understanding of legislation related to children leaving care, care planning regulations, and guidance.
  • Experience or desire to develop management skills, with applications welcome from aspiring or current managers.
