Overview COME BUILD YOUR CAREER WITH VIBRA HEALTHCARE! We are seeking a Full Time MDS/MMQ Coordinator to join our team! Hospital Details The Meadows of Central Massachusetts is an 82-bed transitional care facility located alongside Vibra Hospital of Western Massachusetts – Central Campus. The Meadows provides comprehensive skilled nursing and rehabilitation services for individuals in need of specialized medical care before they can return home or to another care setting. Many of our patients have suffered serious injuries or illnesses. As a result, they require day-to-day help while they work toward recovery and regain their independence. Our comprehensive, personalized approach to patient care incorporates therapeutic intervention and medical management. We specialize in providing short-term rehabilitation, long-term care, and respite care services. Responsibilities Responsible for the development and completion of the resident assessment and care planning in accordance with state and federal requirements, and policies and procedures of the facility. Required Skills: Current, valid, and active license to practice as a Registered Nurse in the state of employment required. Three (3) years of experience in a healthcare setting required. Current BLS certification from a Vibra-approved vendor required. Additional Qualifications/Skills: One (1) year of skilled nursing and/or rehabilitation experience preferred. One (1) year of experience in MDS/MMQ Software preferred. Ability to project a professional image. Knowledge of regulatory standards and compliance requirements. Strong organizational, prioritizing and analytical skills. Ability to make independent decisions when circumstances warrant. Working knowledge of computer and software applications used in job functions. Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
