What you will do:
- Respond to requests for Care Act assessments, Mental Capacity assessments, support planning, and reviews.
- Handle duty work including hospital discharge planning, ensuring a smooth transition for patients returning to their communities.
- Collaborate with health professionals and local services to manage risks and prevent unnecessary hospital admissions.
- Participate in supervisions, both individually with the team manager and in group settings, to enhance your practice.
- Facilitate informal support and sharing of best practices through daily team huddles and interactions with senior social workers.
