This is an exciting opportunity for an experienced HCA who can add their physical health skills to compliment and support the wider older adult community mental health team here covering the Solihull area . You will be supported to provide a unique service within the clinic environment and within service user\’s homes.
This post holder completes physical health tests and monitoring for commencement of medications inc anti dementia medications and provide nursing support to the MDT. Completes home visits.

The post holder will report to care coordinator/lead clinician and GP any physical health concerns that may require further investigation or intervention
Post holder will contribute to meeting the trusts annual physical health target and any other targets set by the physical health or infection control committees.
