HammondCare Cardiff is currently looking for Part time Carers who want to take their career and professional experience to the next level by joining our Specialist Dementia Care Program.
About Specialist Dementia Care Program (SDCP)
SDCP is a transitional care program offered by Hammond Care in NSW for people who experience severe to very severe BPSD. We Offer a small cottage, an eight bedded home designed by the experts of Dementia Centre to provide a safe environment to meet the specific needs.
- This role works in a team of experienced and highly trained Specialist Dementia Carers, with higher staff to resident ratio’s they provide tailored individual care to each resident including personal and domestic support within the residential cottage.
- You will be working closely with and supported by the team of experts including Psychogeriatrician, Clinical Nurse Consultant, Program Manager, and Dementia Advisors.
- The HammondCare unique model of care places the residents needs as your number one priority, you will help create a home away from home for our residents.
- Training, education and ongoing support through onsite CNC, SDCP educator, Dementia Centre experts and Workplace coaches
- Participate in comprehensive weekly review of residents, as well as the development of behaviour support plans and strategies.
To be successful in this role, you will:
- Unlimited work rights, permanent resident or Australian Citizen – essential
- Hold a Cert lll or Cert lV in Aged Care
- Experience is essential – dementia care, NDIS or complex needs
- Have a genuine passion for serving elderly people living with Dementia & a want to make a difference in their lives
- Value teamwork but also be able to work independently and show initiative
- Have knowledge & experience caring for people living with Dementia & their needs
