HammondCare Cardiff is currently looking for Part-time Carers who want to advance their careers by joining our Specialist Dementia Care Program.
About the Specialist Dementia Care Program (SDCP):
SDCP is a transitional care program offered by Hammond Care in NSW for individuals experiencing severe to very severe BPSD. We provide a small cottage, an eight-bed home designed by Dementia Centre experts to offer a safe environment tailored to specific needs.
- This role involves working in a team of experienced and highly trained Specialist Dementia Carers, with higher staff-to-resident ratios, providing personalized care including personal and domestic support within the residential cottage.
- You will work closely with a team of experts including Psychogeriatricians, Clinical Nurse Consultants, Program Managers, and Dementia Advisors.
- The HammondCare model prioritizes residents\’ needs, helping create a home-like environment for our residents.
- Training, education, and ongoing support are provided through onsite CNCs, SDCP educators, Dementia Centre experts, and workplace coaches.
- Participate in weekly reviews of residents and contribute to developing behavior support plans and strategies.
To succeed in this role, you should:
- Have unlimited work rights, be a permanent resident or Australian Citizen – essential
- Hold a Cert III or Cert IV in Aged Care
- Have experience in dementia care, NDIS, or managing complex needs – essential
- Be passionate about serving elderly people with Dementia and making a difference in their lives
- Value teamwork but also be capable of working independently and showing initiative
- Possess knowledge and experience caring for people living with Dementia and their needs
