Licensed Practical Nurse (LPN)

Express is currently hiring dedicated Licensed Practical Nurses (LPNs) to join a compassionate care team serving residents in a long-term care setting. We offer flexible scheduling options including full-time and part-time roles on day, evening, or overnight shifts-with every other weekend required.
Essential Duties and Responsibilities
- Provide hands-on care as part of an interdisciplinary team, assessing and adapting care to meet the individual needs of residents.
- Deliver care in alignment with established policies, procedures, and professional standards-prioritizing resident dignity, independence, safety, and privacy.
- Administer medications and treatments; promptly report changes in residents' conditions and initiate appropriate interventions.
- Collaborate with Registered Nurses (RNs) and the Director of Nursing (DON) to coordinate resident care and communicate with families, physicians, and other relevant parties as needed.
- Complete and maintain accurate clinical documentation, including care plans, daily charting, medication administration records, and lab results.
- Support new staff orientation and ongoing training; participate in performance management when necessary.
- Supervise Certified Nursing Assistants (CNAs) and Certified Medication Aides (CMAs), ensuring high standards of care and clear understanding of duties.
- Assign CNAs based on resident needs, diagnoses, acuity levels, and overall care requirements.
Qualifications
Education & Experience:
- Postsecondary certificate or Associate's Degree in Nursing from an accredited program required.
- Prior experience in long-term care or similar healthcare environment preferred.
Licensure & Certification:
- Current LPN license in good standing with the Minnesota Board of Nursing.
- Current CPR certification.
Skills & Abilities

- Strong interpersonal skills to foster positive relationships with residents, families, and coworkers.
- Skilled in organization, prioritization, and attention to detail.

Physical Requirements
- Ability to sit at a computer and perform desk work for extended periods.
- Frequent standing, bending, walking, and movement required.
- Must be able to lift up to 50 pounds occasionally.
- Physical demands include stooping, pulling, pushing, grasping, and other repetitive motions.
