Job Description
We are seeking a detail-oriented andexperienced MDS Assessor to join our team in Brooklyn, NY. This role isresponsible for the accurate and timely completion of Minimum Data Set (MDS)assessments in accordance with federal and state regulations, as well asfacility policies. The ideal candidate will have a strong clinical backgroundand a thorough understanding of MDS processes and care planning.
Key Responsibilities:
- Complete and coordinate MDS assessments for all residents per required timelines
- Ensure accuracy and compliance with state, federal, and facility guidelines
- Collaborate with interdisciplinary teams for comprehensive care planning
- Maintain thorough and up-to-date clinical documentation
- Participate in quality improvement initiatives related to MDS and resident care outcomes
- Monitor and report any changes in resident status impacting MDS data
Qualifications:
- Bachelor’s Degree in Nursing (BSN) – Required
- Licensed Registered Nurse (RN) in New York State – Required
- Minimum 5 years of clinical nursing experience
- At least 2 years of direct MDS assessment experience – Required
- In-depth knowledge of RUGs, PDPM, and MDS 3.0
- Strong analytical, communication, and organizational skills
- Familiarity with long-term care regulations and CMS guidelines
