Job Description:
Sunbelt Staffing is seeking an experienced Home Health RN to join a top healthcare provider in Jeffersonville, IN. This position requires strong home health experience, proficiency in OASIS documentation, and familiarity with Homecare Homebase. Key Responsibilities:
- Perform comprehensive in-home assessments and develop personalized care plans
- Provide skilled nursing care, including wound care, IV therapy, and chronic disease management
- Complete OASIS documentation accurately and efficiently using Homecare Homebase
- Communicate effectively with patients, families, and the healthcare team to ensure optimal care outcomes
- Participate in on-call duties as needed
Qualifications:
- Active RN license in Indiana
- Minimum of 2 years of recent home health experience
- Proficiency in OASIS and Homecare Homebase required
- Current BLS certification (AHA)
- Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
