A bit about us:
We are a local non-profit healthcare organization looking to grow our team, and add a Home Health/Hospice RN. This is a full time role, 40 hours a week. The work hours are Monday through Friday (day shift) 40 hours a week.
You have the option to work in one of our hospice facilities, or provide home health to our patients in their personal residential homes. Some of the duties include:
- Take vitals as needed
- Collaborate with healthcare team
- Supervise LPNs
- Provide education to patients
- Chart / utilize our EMR system
- Create nutritional plans for patients

Are you a passionate, dedicated, and skilled Registered Nurse (RN) who loves to provide care to those who need it most? If you are, then we have an exciting opportunity for you! We are seeking a Permanent Home Health RN to join our dynamic healthcare team. This position offers a unique opportunity to provide one-on-one, personalized care to patients in the comfort of their homes. As a Home Health RN, you will be a vital member of our healthcare team, providing skilled nursing care to our patients with a focus on geriatrics and hospice care.
You also have the option to work in one of our hospice clinics if you prefer not to travel to a patient's home.
Responsibilities:
As a Home Health RN, your primary responsibilities will include:
1. Performing initial client assessments and re-assessments to determine the patient's needs and suitability for home care.
2. Developing and implementing individualized care plans in accordance with physician's orders.
3. Providing skilled nursing care to patients, including administering medication, wound care, IV therapy, and other treatments as necessary.
4. Monitoring the patient's condition and communicating any changes to the healthcare team.
5. Educating patients and their families on disease management, self-care, and home safety measures.
6. Coordinating with other healthcare professionals, such as therapists and social workers, to ensure comprehensive care.
7. Documenting patient care and progress towards goals in a timely and accurate manner.
8. Adhering to all company policies and procedures, as well as state and federal regulations regarding home health care.
Qualifications:
To qualify for this role, you should have:
1. A current, unrestricted RN license in the state of practice.
2. A minimum of 1 year of nursing experience is preferred, with an interest in home health, hospice, or geriatric care.
3. Experience in a skilled nursing facility (SNF) or providing skilled care is highly desirable.
4. Excellent communication and interpersonal skills, with the ability to empathize with and provide emotional support to patients and their families.
5. Strong clinical skills, with a thorough knowledge of nursing principles and procedures.
6. The ability to work independently, manage your time effectively, and handle multiple tasks simultaneously.
7. A valid driver's license and reliable transportation, as this position requires travel to patient homes.
8. A commitment to providing the highest quality care to our patients.
