Key Responsibilities:

Conduct patient assessments and develop individualized care plans

Provide skilled nursing care, including wound care, IV therapy, and medication management

Document patient progress accurately using OASIS and Homecare Homebase

Educate patients and families on disease management and home care procedures

Participate in on-call rotation as needed
Qualifications:

Active RN license in North Carolina

Minimum of 2 years of recent home health experience

Proficiency in OASIS and Homecare Homebase

Current BLS certification (AHA)

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
