Sunbelt Staffing is actively seeking a skilled Home Health RN to join a dedicated healthcare team in Plains, PA. This role requires expertise in OASIS documentation and Homecare Homebase, allowing you to provide high-quality patient care in a home setting. Key Responsibilities:
- Conduct in-home patient assessments and develop individualized care plans
- Provide skilled nursing services, including wound care, IV therapy, and chronic disease management
- Document patient progress using OASIS and Homecare Homebase
- Educate patients and families on treatment plans and self-care techniques
- Participate in on-call coverage as needed
Qualifications:
- Active RN license in Pennsylvania
- At least 2 years of recent home health experience
- Proficiency in OASIS and Homecare Homebase required
- Current BLS certification (AHA)
- Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
