Job Description:
Sunbelt Staffing is hiring a skilled Home Health RN for a 13-week travel contract in Toledo, OH. This is a field-based role requiring independent case management, excellent clinical judgment, and strong documentation skills.
Key Responsibilities:

Provide patient care visits and assessments in the home setting

Maintain accurate documentation through Homecare Homebase (HCHB)

Complete timely and thorough OASIS assessments

Educate patients and caregivers on disease processes and medication

Coordinate care with interdisciplinary teams and respond to changes in condition
Qualifications:

Active Ohio RN license

Minimum of 2 years of recent home health experience

Required: Experience with OASIS and Homecare Homebase (HCHB)

BLS Certification (AHA)

Valid drivers license, auto insurance, and dependable vehicle

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
