Job Details
- Deliver direct patient care and education across primary and specialty departments
- Supervise LPNs and Medical Assistants, provide performance evaluations
- Coordinate complex care planning and case management for high-need patients
- Handle triage, intake assessments, injections, and phlebotomy
- Collaborate with providers in daily huddles and patient scheduling
- Manage patient follow-ups, pre-op orders, and respond to portal inquiries
- Lead or support health education group sessions and quality improvement projects
Skills You Need
- NYS Registered Nurse license (BSN preferred)
- At least 1 year of ambulatory care experience
- Bilingual in Spanish and proficiency with EMR systems
- Strong interpersonal, communication, and collaborative skills
