What You’ll Do:
- Help patients regainindependence in daily living activities like grooming, dressing, toileting,bathing, and meal prep
- Train patients andcaregivers to use adaptive equipment and improve function
- Promote holistic well-beingthrough community reintegration and meaningful leisure activities
- Conduct in-home assessmentsto ensure safe environments
- Collaborate with care teams,physicians, and families to support patient recovery
