Job Summary

We are seeking to recruit a newly qualified or experienced Band 5 community nurse to join our team. We want you to be a person who values development, be self led and confident. This post is part time 22.5 hours per week covering maternity leave.

Main duties of the job

Our care community teams is based in The Fountains Health Center covering the surrounding geographic area. It\\\’s important that you can drive and have access to your own vehicle for work. But of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
