Job Title: Health and Wellbeing Community Coach x 2

Salary: C1 *GBP*27,711 – *GBP*30,060 (pro rata for part-time role)

Hours: 1 x 37 hours & 1 x 27 hours

Contract Type: Funded until 31st March 2026

We aim to recruit enthusiastic and motivated fitness professionals to support the Active Leeds Health Programmes Team in delivering various exercise and physical activity programmes. Working closely with Adults Care and Health providers, the team promotes self-management, independence, and healthier lifestyles.

Purpose of the Job:

Supporting delivery of NHS-funded health programmes including:

  • Heart and Lungs Community Cardiac and Respiratory Programmes
  • Vocational Rehabilitation Programme (Cardiac, Stroke, and Respiratory)
  • Falls Prevention Strength and Balance Programme
  • Keep Moving

The successful candidate will work with healthcare professionals across venues such as leisure and community centres, ensuring a positive referral experience and providing motivation and support to participants, with sensitivity when needed.

Duties include:

  • Managing referrals and facilitating access to activities
  • Delivering or supporting group exercise classes
  • Creating bespoke individual programmes for clients managing health conditions
  • Promoting health messages and delivering assessments and workshops
  • Monitoring and evaluating programme outcomes

Skills include:

  • Effective communication with diverse groups
  • Health coaching and motivational skills

Qualifications:

  • Level 2 Fitness or Group Exercise Qualification endorsed by CIMSPA
  • Willingness to work towards Level 3 Exercise Referral Qualification, CPR/First Aid, and other relevant qualifications
  • All qualifications must be endorsed by CIMSPA
  • Opportunities for training in Level 4 Fitness and behavioural change techniques
  • Minimum of one year experience in the Health & Fitness industry

This role is based in the UK. Candidates must demonstrate their right to work in the UK before employment. Sponsorship options are available for eligible candidates, typically requiring a salary of at least *GBP*38,700 per year.

Job Purpose:

  • Deliver physical activity interventions to improve wellbeing using evidence-based techniques
  • Support referrals to increase participation and reduce barriers
  • Provide personalized support for physical activity and rehabilitation
  • Monitor and evaluate programmes for commissioners

Responsibilities:

  • Assess referrals and co-produce tailored activity or rehabilitation plans
  • Deliver structured exercise sessions and promote healthy lifestyles
  • Use behaviour change techniques to support lifestyle modifications
  • Gather and report on programme outcomes
  • Engage in ongoing professional development and training
  • Utilize digital channels to promote active lifestyles
  • Perform additional duties as assigned, adhering to council policies

Qualifications: Qualified Fitness Professional at Level 3 endorsed by CIMSPA

Essential requirements:

  • Strong interpersonal and communication skills
  • Experience in behaviour change techniques and working with health professionals
  • Ability to work independently and as part of a team
  • Knowledge of physical activity benefits and health coaching
  • Experience in planning and leading exercise sessions

Desirable requirements:

  • Group exercise instructing skills
  • Knowledge of referral pathways and long-term condition management
  • Level 4 Fitness Qualifications (e.g., Cardiac or Pulmonary Rehab)
  • Additional group exercise qualifications
  • Experience in programme evaluation and effective documentation
  • Full driving licence or ability to travel within Leeds
