Job Summary
Single Point of Access is looking for experienced nurses who are highly motivated, autonomous and have excellent communication skills. The role is a non‑patient facing position in a call centre setting.
Main duties of the job
The post holder will coordinate and manage patients with unplanned care needs, ensuring they are directed to the most appropriate service. Responsibilities include triaging patients via telephone through the Clinical Assessment Service, facilitating appropriate hospital admissions, preventing avoidable admissions, and supporting seamless transitions across primary, secondary and community care settings.
