About the Role:

Liquid Personnel is seeking an Adult Hospital/Duty Social Worker for our client in Wolverhampton. In this role, you will be part of a fast-paced, multidisciplinary team supporting adults at key transition points in their care journey. Based within hospital settings or operating as part of the duty team, your responsibilities will include facilitating safe and timely hospital discharges, safeguarding, and promoting independence through strengths-based interventions. You will collaborate closely with National Health Service (NHS) professionals, care providers, and community partners to ensure appropriate support is in place – while upholding the principles of the Care Act 2014, personalisation and reablement.

What will your responsibilities be?

In this role, you will

  • CompleteCare Act assessmentsto determine eligible care and support needs for patients ready for discharge.
  • Develop and implementstrengths-based support plansin collaboration with the individual, their family, and professionals.
  • Work effectively within amulti-agency teamincluding hospital discharge coordinators, nurses, OTs, and physiotherapists.
  • Undertakesafeguarding enquiriesunder Section 42 where appropriate, ensuring adult protection procedures are followed.
  • Carry outMental Capacity Act assessments, and contribute tobest interest decisionswhen capacity is in question.
  • Supporttimely discharge planning, ensuring that delayed transfers of care are minimised.
  • Respond tourgent or unplanned referralsvia the duty system, triaging and prioritising cases based on presenting risk.
  • Promote the use ofcommunity-based resources,reablement, andpreventative servicesto avoid unnecessary readmission.
  • Maintain high-quality, accurate, and timelycase recordsusing systems such asMosaic.
  • Contribute to regularreviews and reassessmentsto ensure that support remains proportionate and effective.

Qualifications & Experience:

To be successful in this role you must have,

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered withSocial Work England.
  • Experience: Substantial experience working with vulnerable adults, including those with complex needs.
  • Legislation Knowledge: Strong understanding of theCare Act 2014,Mental Capacity Act 2005, and safeguarding procedures.
  • Skills: Excellent assessment, analysis, and communication skills. Ability to work collaboratively in a multi-disciplinary setting.
  • Driving Licence: Full UK driving licence and access to a car is often required for community visits.
