Job overview

We are looking for passionate and skilled Band 5 Community Nurses to join our Thanet Community Nursing Teams, to be based at our brand new £10million state-of-the-art Thanet Health Hub.

Our new hub directly supports the Governments new 10-year plan, that places community care at the heart of our health system.

This plan represents one of the most significant shifts in NHS history: moving care from hospitals into neighbourhoods, from reactive treatment to proactive prevention. And here at Kent Community Health, we\’re right at the forefront.

Planning to open in Autumn 2025, our new Thanet Health Hub will consist of an NHS community diagnostic centre, a range of community NHS services, including community nursing, relocation of an existing GP surgery and signposting and support to access health and care services provided by voluntary sector organisations, such as Age UK.

Main duties of the job

Community nurses provide complex nursing care to people at home and in residential care homes.

As a community nurse, you\’llassess the healthcare needs of patients and families monitor the quality of care they\’re receiving be professionally accountable for its delivery.

Your patients could be any age, but they\’ll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities

Qualifications

  • Registered Healthcare Professional with relevant Professional body (1st Level Nurse/ NMC). Degree in a health relevant subject or willingness to complete a degree (within 3 years of appointment).

Experience

  • Experience of recent community/acute healthcare gained through education or work experience placement or previous employment.

Knowledge

  • Awareness of professional issues and recent developments in the NHS. Knowledge of up to date clinical practice.

Skills & Abilities

  • Ability to organise own work
  • Sound communication and interpersonal skills (verbal and written)

Personal Attributes

  • Ability to motivate and organise others to ensure best practice.
  • Valid UK driving licence and access to a car for work purposes.

Ability to Manage

  • Ability to prioritise care delivery in a professional manner, taking into account Trust policies and procedures
  • Able to support the implementation of local and national agenda for health under the guidance of the team lead / ward manager
