Social Worker – Adult and Learning Disability – Perm – up to £42,840 – hybrid / South London – Qualified A South London Council is excited to offer a rewarding opportunity for a dedicated, experienced, and qualified social worker to join their Adult and Learning Disability Services team. This permanent position presents an excellent chance to make a significant impact on the lives of young people with additional needs and their families, helping them transition into adulthood. The role is part of our dynamic and supportive team, where you\’ll collaborate closely with Children\’s Services, Education, and Health to ensure that young individuals receive the right support as they prepare for adulthood. The focus of this role is primarily on supporting the assessment and support planning for high-cost placements and care packages for young people approaching adulthood who have learning disabilities. You will be responsible for conducting Care Act 2014 compliant eligibility assessments for young people and their carers. This requires a timely identification of social care needs and the creation of person-centered, strengths-based outcomes within support plans.
