The post involves collaboration with front door services, respiratory hot clinics, virtual wards, community stakeholders, outreach services, and outpatient respiratory services.

We are seeking proactive specialist clinicians dedicated to improving patient experience, optimizing care to support acute hospital admission avoidance, prevent re-admissions, and promote effective self-management strategies for patients and their carers.

Ongoing training and development opportunities will be provided, supporting both specialist and trainee specialist posts, in collaboration with existing multidisciplinary and education teams.

  6. Implement peer reviews and evaluations of safety, quality, and health outcomes from pathway to system levels.
  7. Provide high-quality, values-based professional practice that enhances patient experience and supports decision-making in complex situations.
