The Head of General Practice Integrated Development is a key role in delivering Lambeth\’s transformational change programme. The post involves working alongside Primary Care system leaders to support and develop an organisational and governance structure that enables Primary Care to participate fully in the integration at place agenda. This at-scale General Practice organisation will support resilient General Practice and influence the role of General Practice within integrated neighbourhood team working.

Interviews are expected w/c 11th August.

Lambeth Together is an integrated local care partnership aimed at improving access to health and care services for Lambeth residents, helping them lead healthier lives. The post holder will develop strategic and operational plans for implementing the Primary Care Development Programme across Lambeth, collaborating with colleagues across the local health economy.

Lambeth has a strong record of system-level change through the Lambeth Together Partnership and Delivery Alliances, focusing on population health and wellbeing improvements.

Our Delivery Alliances include:

  • The Living Well Network Alliance – supporting adults experiencing mental illness or distress.
  • The Neighbourhood and Wellbeing Alliance – improving health and wellbeing in local neighbourhoods.
  • The Children and Young People Alliance – supporting children and young people to grow up healthy and happy.

This role is within the Primary Care and Transformation Team, responsible for transforming Primary Care services beyond national core contracts, and collaborates with Alliances that bring together partners and those with lived experience to plan and manage programmes aimed at service integration and health inequalities across Lambeth.
