What about the Team?
This is a new role in the Adult Social Care & Prevention Directorate sitting within Adult Social CarePoint.
As a key portfolio priority for the coming year, we continue to develop our preventative approach to all interactions with people with care and support needs, to promote independence and ensure the most appropriate, cost effective and least restrictive care.
The role is being created to support this approach, working as part of a multi-disciplinary team at the Directorate’s front door, Adult Social CarePoint (ASCP). ASCP offers a multi-disciplined, responsive service, with the ability to visit people where they are and fully understand their circumstances. This enables us to make prompt interventions, such as equipment and short-term support to help people regain skills and confidence. Through this rapid response, we aim to reduce or negate the need for ongoing support.
What about the Role?
As a Moving and Handling Specialist you will provide Moving and Handling support to meet the identified needs of people presenting at the ASCP, working as part of a multi-disciplinary team (MDT) to support people to maintain independence, prevent, reduce, or delay care needs, and avoid unnecessary admissions to acute care and residential or nursing care.
As a key member of the MDT, you will work alongside a team of Social Workers, Occupational Therapists, a Physiotherapist, a Falls Prevention Coordinator, Reablement, and health professionals to contribute clinical expertise to holistic assessments and short-term interventions. You will help identify functional and mobility needs, advise on equipment and adaptations, and develop person-centred plans that empower individuals to regain or maintain independence in their own homes.
This front facing position offers the opportunity to make a real impact at the first point of contact, supporting strengths-based approaches and ensuring timely, appropriate care pathways for adults across a wide range of presentations, including falls, frailty, long-term conditions and hospital discharges.
