The predicted salary is between 36000 - 60000 £ per year.

The focus of this role will be within the inpatient setting, optimising the management and delivery of heart failure care to adults admitted to the Trust with Heart Failure, reviewing patients at the point of diagnosis and also those who are known Heart failure, admitted with decompensation. You will be required to use advanced specialist skills and knowledge to undertake full assessments of patients with Heart Failure, leading to safe decision making and implementation of management plans. Providing effective, evidenced based treatment, improving patients’ quality of life through delivering high-standard and holistic care. There will be participation in heart failure doctor-led wards rounds but also autonomous reviews of patients will be required.

You will run a nurse-led 2 week post discharge clinic to capture patients who have been discharged after an acute heart failure admission, in line with national standards and to reduce re-admission rates.

Excellent communication skills are required and the ability to work as part of a multi-disciplinary team is essential, liaising with all health professionals treating heart failure patients in primary, secondary care and tertiary centres to ensure seamless care.

To actively participate in multidisciplinary team meetings

To take part in developing heart failure services and clinical practice through involvement in management, teaching, education, research and audit activity.

Our friendly and well established heart failure service in Croydon is looking for a highly motivated nurse to join our team in this large and diverse London borough.

We are a pro-active and innovative multidisciplinary team, working across community sites and within Croydon University Hospital. We are currently running some exciting service development projects and strive to deliver high quality care to all our heart failure patients.

If you have heart failure experience with excellent organisational and communication skills and would like to join our team, we would love to hear from you. The ideal candidate would be an experienced heart failure nurse, qualified in physical examination and non- medical prescribing, however professional development will be supported.

The successful candidate must be committed to working with heart failure patients, providing excellent standards of care and being an integral member of the multidisciplinary team.
