Band 6 – £37,338 – £44,962 (pro-rata)
Hours per week: 27.75 (The service runs 0800-1800 across 7 days)

Responsibilities

The Integrated Transfer of Care (ITOC) Teamis a dynamic and multi-skilled team consisting of nurses, occupational therapists, physiotherapists, and Assistant Practitioners. We also work in alignment with social services and other Locala community services.

Our purpose is to support a safe and timely hospital discharge of people who are medically fit, and no longer need hospital care, to a place more suitable to the person’s needs.

The main priority of the team is to consider home first with ongoing community interventions, if required.

We are now looking to appoint an experienced Occupational Therapist to our team based in Kirklees. We offer a high degree of flexible working within the role, whilst ensuring that there is cover over the 7 days.

Key Requirements:

General / Specialist Knowledge

• HCPC Registered

• Awareness of implications of Clinical Governance

• Evidence of continuous professional development up to diploma level or

equivalent

• Involvement in audit projects/evaluating service

• Experience of supervision of support staff

• Awareness of Intermediate and Urgent care agenda

• Working knowledge of broad range of patient conditions

• Experience of working as part of a rehabilitation and/or multi-disciplinary team

• Knowledge of Integration of services

• Evidence of leading and managing staff and caseload management

• Experience of working within a multidisciplinary setting

• Experience of working collaboratively with other agencies

• Experience of working in community services

• Clinical Educator Course

• Knowledge of inter-disciplinary working

• Discharge planning.

Skills / Abilities

• Good organisational skills

• Ability to identify potential for rehabilitation through assessment

• Ability to prioritise and manage own caseload

• Be able to use research, reasoning and problem-solving skills to determine treatment

• Ability to use a range of IT systems

• Able to promote Service with support from supervisor

• Excellent interpersonal and communication skills

• Negotiation and conflict management skills

• Team leadership experience/qualification

• Clinical supervision qualification

• Ability to manage competency
