Specialist Support Worker

About the role
We are seeking a Specialist Mental Health Support Worker for our supported living rehabilitation service in Shropshire. Based in Shrewsbury, you will provide rehabilitation-focused support to individuals with complex and enduring mental health needs as part of a 24-hour service. You will support customers with life skills, promote independence, and help them build relationships within their community, supporting their health, well-being, and tenancy management.
Key Responsibilities
- Deliver high standards of support, demonstrating good practice.
- Promote independence and choice for each individual.
- Assist with personal hygiene and appearance as needed.
- Support social, leisure, work, and educational activities, including access to ICT.
- Help with meal preparation, menu planning, and budgeting.
- Encourage community participation.
- Support clients with moving on from services, liaising with care teams.
- Establish links with healthcare professionals and agencies.
- Undertake key-working, including support plan reviews and meetings.
- Conduct assessments in collaboration with clients and agencies.

About You
We are looking for a motivated individual with experience supporting adults with mental health needs, including self-harm, personality disorder, and schizophrenia. You should be self-motivated, able to work independently, and experienced in partnership working and creating personalized support plans. You must be flexible to work within a 24-hour rota, including evenings, weekends, and sleep-in shifts. The role requires NVQ Level 2 in Health and Social Care and experience in supported housing settings.
