Summary
We are hiring a Home Health Physical Therapist to join our team in Buckhannon, WV, serving patients throughout Randolph, Webster, Lewis, Braxton, and Upshur Counties.
Salary based on full-time employment and maximum productivity: up to $120,000 per year (based on experience)
Bonus available for qualifying full-time employees.

Responsibilities
The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this, they develop treatment plans, perform interventions to improve patient well-being, and evaluate progress.
- Evaluate the patient\’s functional status and physical therapy needs, assisting in developing the therapy plan of care with the physician.
- Observe and report patient responses and condition changes to the supervising nurse or physician.
- Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to accepted standards.
- Instruct and inform healthcare team members, patients, and families about the care plan, limitations, and progress toward goals.
Education and Experience
Licensure Requirements:
- Current Physical Therapy license in the state of practice
- Current CPR certification
- Valid driver\’s license, vehicle insurance, and access to a dependable vehicle
