Job Scope
- Engage patients referred by the healthcare team to participate in community-based activities, either in person or through follow-up phone calls.
- Introduce patients to a range of available community activities, assist them in selecting suitable options, and support their registration process.
- Educate and motivate patients on the value of community participation in improving health and well-being.
- Follow up with patients to check on their attendance and participation in activities.
- Assist with basic navigation support and respond to general queries from patients.
- Facilitate the completion of patient questionnaires when needed.
- Coordinate with community partners to gather and update schedules of ongoing programs.
- Submit sign-up records to partners for tracking and retrieve patient attendance information.
- Maintain accurate records of all patient engagements and referrals.
- Provide timely updates to the clinical team on each patient’s community involvement.
