This is an exciting time for the London Borough of Redbridge; we are at the heart of regeneration and change in East London, with unprecedented investment in the area expected in the next few years and Cross Rail will open up new opportunities for new homes and jobs. We benefit from excellent transport links across London and Essex and our offices, based in Ilford, are only 9 minutes from Stratford and 18 minutes from Liverpool Street, in the heart of the city.
Our vision is Ambitious for Redbridge, and to help us meet our challenges, the Council is shaping its service areas to meet the needs and diversity of the population. As part of the vision we have restructured our Community Health and Social Care Team to develop a new service model and are currently recruiting to a number of new and exciting posts.
The successful candidate will specialise in working with people with dementia, physical and sensory disabilities, learning disabilities and mental health. This will include developing community-based programme of provision based on the findings of community care assessments, person centred plans and risk assessments. Central to this is delivery of, effective, efficient and value for money services.
You will consult with service users, carer&s and families for person centred support plans and key working, which promote independence, including health and wellbeing outcomes, community outcomes, personal empowerment and development, wishes and to try new things.
To assist, develop, implement and monitor activities within the service, including social, recreational, therapeutic and educational which are designed to meet the service users needs.
To undertake escorting or driving duties, transporting service users to and from home and when required for activities or outings. Driving license an advantage.
To assist service users, including those who use wheelchairs and other aids, in safe movements around the day centre and getting on and off vehicles.
Typical daily hours will be between 10am and 3pm, however, times may occasionally vary in accordance with service delivery.
