- Essential Car Documentation: this post is a designated Essential Car User. You must hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance.
- DBS (Disclosure and Barring Service) clearance: this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced DBS check. Information provided will be dealt with confidentially and in accordance with the DBS Code of Practice. Qualification: Hold or be willing to undertake the Care Certificate and minimum level 3 or equivalent qualification, or be willing to work towards this within 6 months of starting the role.

This is an exciting opportunity to join the Building Independence Team within Reablement. This model of care will ensure a dedicated team of trained and skilled support workers, competent in supporting individuals with specialised needs to achieve lifelong, sustainable changes to reach their goals.
The Building Independence Team will offer a more efficient and consistent approach to support, maximising potential for individuals to reduce care needs while maintaining high standards of service delivery.
The Reablement provision will support cohorts while utilising the TEC offer, including:
- Individuals with substance misuse
- Individuals with low to medium mental health needs
- Individuals with learning disabilities
- Individuals with autism
- Individuals with neurodiversity
- Support to residential/nursing provision where 1:1 support is identified, including approaches to manage distressed behaviours and falls
- Support for individuals with early-stage dementia to remain in their own homes
The successful candidates will join a service that prides itself on helping people achieve their goals and return to independence.
The candidate will work in a community setting citywide within an individual’s own home to support them to achieve independence, longer-term solutions and aspirations while maintaining privacy and dignity.
The post holder will provide and promote choice and control to the individual, helping them transition to maximum independence through a person-centred approach and partnership working.
The post holder will maintain accurate, up-to-date records and collect data to evidence performance against service delivery and targets.
