Job Title: Health Facilitation Practitioner

Starting Salary: £27,485 progressing to £30,162 per annum – Band 4 NHS Agenda for Change terms and conditions

Hours: 37.5 hours per week

Location: Countywide

Contract Type: Permanent

Directorate: Adult Care Services

About The Team

The Health Liaison Team supports people with Learning Disabilities who are Admitted to hospital for planned or emergency admission

About The Role

You will be working to support clinical colleagues to understand and implement reasonable adjustments to reduce the risk of health inequalities. You will be co-delivering training to clinical staff with Experts by Experience through structured and informal sessions, along with supporting Student Nurses as part of the wider Health Liaison Team. We would encourage people who are passionate and creative about individual approaches to take on this hugely rewarding role.

About You

Essential Criteria

  • Full driving licence
  • Previous experience working in the field of learning disabilities
  • Previous experience working in the community with people with learning disabilities
  • Knowledge or an Acute Hospital environment and process
  • Experience working with other professionals
  • Experience working as part of a team
