You will undertake holistic needs-led assessments in partnership with service users, patients, and carers. You will prepare care plans, commission services, and manage a caseload to ensure the most effective and beneficial care packages are established, delivered, monitored, and reviewed. All activities will comply with statutory requirements, Torbay and South Devon NHS Foundation Trust policies, standards, and guidelines.
Be a core member of a multi-disciplinary Baywide Health and Wellbeing Team. Contribute to effective assessment and discharge planning with working-age adults in Torbay and for Torbay residents in out-of-area hospitals within the region.
Undertake holistic social care assessments in partnership with service users, families, and carers. Prepare care plans, commission services, and ensure care packages are beneficial and cost-effective, complying with all statutory requirements and Trust policies.
Hold clear responsibilities for safeguarding adults and children, operating in a manner consistent with Human Rights legislation. Practice in compliance with social care legislation, including the Care Act 2014 and Mental Capacity Act 2005.
