We are seeking a dedicated and experienced Senior Occupational Therapist to join the team at one of the NHS Trust in Manchester. The Trust is known for its commitment to high standards of patient care and its focus on continuous improvement and innovation in healthcare services. This role offers a unique opportunity to work in a dynamic environment dedicated to providing high-quality care and rehabilitation services to frail and elderly patients.
Role and Responsibilities:- Deliver high-standard assessments and rehabilitation interventions to patients in an intermediate care setting.
- Develop and implement individualized treatment plans, focusing on maximizing patient independence and safety.
- Work collaboratively with a multidisciplinary team, including nurses, social workers, and other allied health professionals, to provide comprehensive care.
- Manage a diverse caseload, ensuring timely and effective care delivery for community-based and inpatient services.
- Educate and support patients and their families in understanding treatment plans and rehabilitation goals.
- Actively contribute to the development and review of clinical protocols and best practices.
- Diploma/Degree in Occupational Therapy or Physiotherapy.
- HCPC Registration.
- Experience in intermediate care or rehabilitation settings.
- Ability to effectively manage care for frail and elderly patients.
- Strong community experience and a proactive approach to patient care.
- Excellent communication and interpersonal skills.
- Strong clinical skills in assessing and treating a range of conditions.
- Ability to detect the deteriorating patient.
- Ability to risk assess patient assessment, treatment and discharge.
- Ability to assess and treat motor and sensory skills as well as physical functioning.
- Ability to assess and treat cognitive and affective impairment, its impact on function, ability of the patient to maintain their health and safety.
- Ability to structure holistic assessment and the OT process within a framework that provides clear clinical reasoning for recommendations made in relation to a patient's needs and care pathway.
- Evidence of CPD.
