Minimum Qualifications:
- Degree: Master of Social Work (M.S.W.), accredited by CSWE.
- Certification: Current, unrestricted clinical social work license in the U.S. or territories.
- Experience: At least two years in care coordination, discharge planning, or case management in a medical setting within the last four years.
Required Skills/Abilities:
- Manage EFMP-M in accordance with DoD and military policies.
- Use clinical assessment skills for timely identification of families with special medical and educational needs.
- Collaborate with various service providers and ensure proper data management and reporting.
- Coordinate enrollment, travel screening, and maintain documentation for families with special needs.
- Participate in meetings, training, and support the integration of EFMP services.
- Conduct quarterly case reviews and coordinate patient assessments and care plans.
- Follow all applicable policies, complete documentation, and maintain training requirements.
- Apply skills in assessment, planning, facilitation, and advocacy for families.
- Possess knowledge of psychosocial practice, human development, and social systems.
- Work effectively with service members and veterans facing complex issues.
- Maintain confidentiality in accordance with HIPAA and accreditation standards.
- Proficient in Microsoft Office, data management, and effective communication skills.
