Responsibilities:- Perform personal care and attend to patient requests promptly, reporting to the Case Manager if unable to perform a task.
- Communicate positively with patients and families, maintaining confidentiality and ethical standards.
- Ensure safety and proper use of equipment such as foot stools, side rails, and oxygen.
- Provide personal care including baths, oral hygiene, shampoos, and changing linens.
- Assist with dressing, feeding, and mobility as instructed.
- Record vital signs and observe patient skin condition.
- Report changes in patient condition to the Case Manager.
- Assist with medication reminders, light laundry, and simple procedures as ordered.
- Maintain a clean and orderly environment and adhere to documentation standards.
- Participate in quality improvement activities and respect patient and family needs.
- Meet state training requirements with a minimum of 75 hours of classroom and supervised practical training.
- At least six months of experience providing personal care.
- Be at least 18 years old, with the ability to read, follow instructions, and document care.
- Understand hospice philosophy and comfort with terminally ill care.
- Possess current CPR certification, demonstrate tact, patience, good hygiene, and good communication skills.
- Successfully complete a probationary period demonstrating competence in job qualifications.
