We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients. Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing.

Main duties of the job:
- Our care community teams are based in Princeway Health Centre covering Frodsham and surrounding geographic area.
- It is important that you can drive and have access to your own vehicle for work. Mileage expenses are reimbursed in addition to your hourly rate.
- As a Community Staff Nurse, the successful candidate will undertake duties involving the delivery of high-quality nursing services and holistic assessments of patients ensuring a person-centred approach.
- You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home.
- You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
