Your Role:
As the Clinical Lead, you’ll be a key figure in ensuring that every resident receives personalised, respectful, and effective care. You will lead on quality improvement initiatives across the home, champion a resident-first approach, identifying clinical, health, and social care needs, advocate for excellence in care and support resident independence and dignity, and develop and manage individualised care pathways tailored to each resident.
You’ll be responsible for:
- Providing clinical leadership and overseeing all aspects of resident care.
- Conducting comprehensive assessments and creating detailed, person-centred care plans.
- Ensuring compliance with safeguarding, dignity, and clinical governance standards.
- Creating, reviewing, and delivering personalised care plans.
- Managing medication safely and effectively, in line with national and organisational policies.
- Liaising effectively with external professionals to coordinate and deliver care.
- Promoting strong relationships between residents, families, and staff.
