Adult Physical Health services are divided into 4 categories: Anticipatory, Planned and Unplanned Care, and Specialist services. All services aim to improve outcomes for people and maximise access to services through integrated working.
Working alongside the District Nursing Service with an Anticipatory Care focus aims to reduce pressure on planned and unplanned care services. Supporting, teaching, and providing advice and support following best practice guidance is key to the role. The goal is to optimise patient care and ensure that, where possible, patient independence is preserved and maintained.
The Community Teams aim to maintain people within their own homes/communities wherever possible and facilitate earlier discharge from hospital through delivering a more coordinated approach to care. They work closely with colleagues in General Practice and Third Sector organisations. Central to the philosophy and approach of the Community Teams is placing the individual at the centre and proactively maximising opportunities for people to have choice, personalisation, and self-care.
Self Management Facilitators within the Community Teams aim to maintain people within their own homes/communities, clinics, and hospital inreach wherever possible and facilitate earlier discharge from hospital through delivering a more coordinated approach to care. The post holder will work within standard operating procedures/pathways to manage their own workload independently but with reference to a supervisor within the clinical area. In doing so, they will support the assessment of patients and implementation of programmes of care, ensuring that they practice within their sphere of competence and knowledge.
